Here is the 24 page application for WI Medicaid Elderly, Blind and Disabled (http://www.dhs.wisconsin.gov/forms/F1/F10101.pdf). I was pretty healthy, was self-employed and worked more than full time, yet paid for the next 4.5 years between $800 and $1,200 a month. With high deductibles and co-pays, too.
